Does the Document Change Register index need a revision date?
Does the Document Index/ Document Change Register itself have to have a last updated or revision date of it each time a form/ policy is updated?
P.S. Each time an individual form/ policy is updated that particular items receives an updated date but I am not sure about the actual Document Index/ Document Change Register.
I would not give it a new revision date for changes to data. We would only give it a new revision date if we update the register template.
I have never included a date on a document revision register and it has been ok during audits.
In our facility, we only change the revision date for our programs and policies or templates. So for a log or record of information, we don't change the revision date after every entry in the log/record, we only change it if the actual skeleton of the log/record changes. This could be changes in the questions being asked, the scope, or the individuals responsible.
I would see a document revision register as just another log with data being added (or changed, depending how yours is set up).
