Can anyone suggest an effective tool to monitor supplier performances on an ongoing basis?

I am in need of suggestions on what to use to perform a supplier ongoing performance monitoring

a simple way to do this is to create a form that is filled out on a regular basis (monthly or so).   On this form you can evaluate or rate the supplier on areas that are important to your operation.  for instance: documents received / received on time such as audits, LOGs, coas, etc, rejected shipment or other issues with product, trailer issues, late or missing deliveries, internal testing results, etc.   From these results you update the supplier's risk.   

the goal is that that once a supplier is approved you are monitoring their performance periodically and adjusting risk and monitoring as needed.

We have a supplier approval program SOP and a supplier review every 2 years to ensure they are still matching our requirements. Part of the supplier approval process is to have each supplier fill out a questionnaire and provide documentation listed below. We also conduct supplier site visits to ensure they are in line with our standards. We test materials against the COA and also collect all documentation that supports product claims, GF, Kosher, USDA Organic, etc..   

1. Organizational Chart (1.1.A.)
2. Supplier Approval Program (2.1.A.)
3. Lot Number System Explanation (3.1.A.)
4. Any Applicable Certifications and/or Licenses, that you currently Hold (FDA, State, ISO, SQF, AIB, Organic, Etc.)
5. The Results of Your Most Recent GMP, cGMP, or Food Safety Audit. (FDA, USDA, Third Party, or Internal)
6. Your Most Recently Completed Mock Recall

I am into fresh fruits and Vegies. I am looking for supplier ongoing performance review/monitoring. I already have yearly monitoring by obtaining their FS certificate (GFSI); in need of ongoing monitoring tool.

Maybe, use a spreadsheet that records 1. required corrective actions 2. incoming rejections, 3. supplier issues.  These all should already be on your QA Hold log/ or CAPA log. Perhaps you can use a pivot table from your log and then chart that data, even creating a trend line if you want.  You could take it a step further and set a target for your specific suppliers that would trigger "Action Needed"   

You can incorporate a button on your chart that links to your individual suppliers and see just their information. 

For mine, I have two charts.  Chart 1) if I click on suppler "x" it tells me all the issues I had with that supplier based on product issues, receiving issues, and document issues. Well, the ones I've recorded on my CAPA anyway. Chart 2) just gives me a total incident report of all my supplier, based on product. So, I can see all my pork suppliers and see how many total incidents they have had and compare them against my other suppliers. So, when I have a need to increase my orders, I can take those numbers into account. Also, it helps if you do suppler reviews. I do them once every three years, just so they know I am keeping score. 

It really doesn't add any time to your day if you already record this information, besides your initial formulation creation, then it's all automatic.   

If you don't record this information, or it's on some other electronic source it might take more time.

Not sure if this will help, but I have been using a task manager ( I use monday.com) and i am able to add each supplier & their requirements and set the review on a regular schedule based on risk. The website automatically notifies me that a review is required, and it allows me to add notes and upload new documents as they expire, etc. Hope this helps, let me know if you need more details! 

I was looking for infos about monday.com and I found your post. Are you full satisfied ? (what about the price ? I think that with few users this is OK but it seems to be very expensive if you scale, agree ?
