Does anyone have a new ingredient template/format that would be willing to share?
Sorry did you mean the matrix for raw material?... or change management procedure if there is a new RM?

I am looking to implement a document to approve the raw ingredient in purchasing. Currently we do a separate risk assessment for the vendors & hazard analysis per the raw material grouping - based on the HA & Risk Based Preventative Controls Guidance per the FDA.
We are a small dry blending SQF Certified facility.
If sqf, this is outlined in 2.3. the clauses are designed for new products but also mention changes to ingredients.
Depending on what the ingredient is used for, it may not make sense to approve an ingredient without going through the entire process.
I have attached a basic form that I have used for approving new formulas / updating formulas.
or maybe im not understanding what you are trying to achieve. let me know.
