BRCGS Haulier Transport Questionnaire

Hello! 

 

Apologies if this has been answered, I couldn't find anything specific. Currently working towards A&B BRCGS. We have a robust SAQ, but I not a Transport/Haulier questionnaire.

 

We mainly transport Frozen and ambient sauces. 

 

Can anyone recommend a template of one that I can use as a guide please?
