Bonita RUSSELL : We always audit the person(s) doing the activities, not the manager of the department to ensure training was effective and SOP is being followed.  MGT is audited for their supervisory activities and technical knowledge.

Bonita RUSSELL : Ours is well prepared, all information on the agenda prior to the meeting so we all know what information to bring.  Also, we have added many supervisors as well as Managers for monthly meetings.  We keep HR and financial issues to Senior Management.  Our audits take about 3 hrs and always includes lunch.  It also takes me about an hr to review the agenda, collect any information I might need, and send new issues to Sr. Management to add to agenda.

Bonita RUSSELL : So, auditing, completing complaints, reject root cause anaysis, other tracked items are NOT completed until they are logged into the trend/log.  This makes information up do date at all times.

Bonita RUSSELL : We do not use checklists, the auditor writes up what they saw/reviewed/what interviewers said... etc.  How you write this up is up to you, we allow the auditor to write directly on the Uncontrolled SOP/program they are auditing, this is then kept as documentation.  If it's it too neat was it even done.  When I worked in a preclinical laboratory, the Pathologist told us, if there is no blood on the necropsy forms, did you actually do it!

Bonita RUSSELL : An example of working on an audit that will take a long time to finish.  We do sometimes have audit findings that require new equipment, new training, validations... That can take more than the current year.  how do we document that well?
