Understanding Costco FSVP Requirements for Supplier Approval

Below is an Expert from the Costco food safety requirement manual.

 

"Costco US has implemented a Foreign Supplier Verification Program that requires all imported items subject to FSVP per FDA, and regardless of the

importer of record, be verified and issued a letter of compliance by Costco’s qualified individual. The approval letter is specific for the item(s) and facility that produces the item(s)"

 

Questions

1. How to get a Costco FSVP approval letter? 

2. Who is a Costco Qualified individual? 

3. If i buy an imported ingredient from a US based distributor, do I need to get a Costco FSVP letter for those ingredients?
