How to Analyze and Conclude a Food Safety Culture Survey?

please help me in how to measure a survey.
i have prepped the questionnaire but how to conclude it?
I usually have each question answered by a number rating. Then average the answer to each question. Set a baseline of the minimum required average. for those question(s) that fall below the required average - corrective actions shall be developed for improvment in the area represented by the questions.
If you are using Microsoft applications I would suggest using MS Forms. It will auto trend your information so you can focus on the bigger ticket items. Also can utilize anonymity if needed, as anyone with the QR code or link can take the survey on any device. I use MS Forms for quite a lot of things, including training quizzes as you can assign each question with a grade as well.
I have this excel sheet you can use:
first give the trainings and assess them:
OPERATOR GMP Hygiene & Cleaning Halal CCP & OPRP
- Initial training
- Assessment
- Follow Up training
- Assessment
- Focused Training
- Final Score
then give the main score using this - the assessment should be based on the criteria below so cover all in the training
A- Proficient/ Excellent knowledge
B-Moderate knowledge
C- Basic knowledge
- FSMS Knowledge/awareness
- Completion of induction program
- Ability to understand and interpret rules and instructions
- Ability to follow instructions Knowledge in area of work (Educational qualifications/ trainings) Food safety culture
- Ability to understand food safety and hygiene concepts
- Ability to follow food safety and hygiene rules
- Ability to communicate within the team
- Ability to report food safety issues to concerned supervisors
- Ability to comply with cleanliness and hygiene standards
- Knowledge in personal hygiene standards
- Knowledge in OPRP/CCP monitoring
- Knowledge in cleaning programs GMP Awareness level
- Ability to manage outsourced staff

I'd only place a caveat to all of this home grown survey stuff. There is a lot of fancy stats, I have to admit I don't fully understand that goes into the design and structure of getting difficult to measure aspects like culture measured accurately. That includes things like making sure it is actually capable of measuring what it says and compensates for a site where employees will (deliberately or accidentally) be over optimistic about compliance.
I'm not saying these home made things don't comply with GFSI standards but if you want it to be meaningful, you might need to use something a bit more robust.
