Does anyone know what the specifics are around a phone number on a label?

Does anyone know what the specifics are around a phone number on a label? We are a dietary supplement contract manufacturer and our customer has their phone number on the label as required for being the owner of the product but it is written both alpha and numerically (1-800-FDA for example) is that allowed?

Thats what i believed as well but i recently tried to submit for a certificate of free sale through our industry system for this company and it was rejected for because they said the customers label does not have a complete u.s. based street address AND/OR phone number. The customer only lists the city, state and zip code on the label but they do have a phone number but its alpha-numeric

If some automated system is rejecting it, just convert it to numerals when you type it in.
