FSSC Supplier Questionnaires

Please assist  - is the below correct ?

all materials used, all packaging used and all chemicals and all ingredients used on your site should have a supplier questionnaire filled in from whom you purchase it right ?

Not just the company supplying the eg. chemicals filling in a overall questionnaire - they must do it per chemical they supply right ? This is for a food production plant - medium to high risk.

If you mean even the manufacturers of cleaning chemicals should have a questionnaire, then no. 

A general supplier questionnaire should cover the supplier and all of their products. We don't have our chemical supplier sign an agreement per chemical; though they send letters of guarantee for the product.
