Software change management procedure

Good day friends,

Does any one have the procedure on managing the software changes in food industry? As my company have full automation, we do lot many software changes i.e. troubleshooting and upgradation from time to time. Now, we want to introduce a system where any software change gets into a systematic flow process and gets documented whenever it is changed.
