How to conduct and record "On the job" Training?

Hello,
I was wondering, what is the best way to do "On the Job Training" and the refresher training on that. Would it be fair to create the Questionaire where we would ask is the person is able to do the list of the things, and then the senior emploee or his manager would sign this form. From this form we could see what skills the person has to improve or to learn something.
This is the only one idea which I have how we could do On the job training.
Would you mind to suggest and to share your experience with this.

Observation and questioning are a good combination to find out the employees level of competence. You must have some benchmark or standard that you can compare against such as a Standard Operating Procedure and the person doing the assessment must be competent and equally have a good way about them.

I have attached a generic template/training protocol for how I manage lab based on-the-job training - it may be of some help and is easily adaptable to any position. It needs to be written though by or in conjunction with someone who knows the job function that is the subject to the training exercise. Could you just let me know are you doing the similar training to the office workers as well? what about the other skills like ability to drive the forklift? or ability to make decicions when necessary...? I think would be great to add all the questions, skills like that. What do you think?
are you completing that form annualy?

Typically this is completed when someone new comes to a job but there is no issue in completing it for employees that are there a while but have no formal documented training. It can be applied to any position although I personnally have not used it for office workers. glad to help
