Without knowing the product you would be recalling, you can find some press releases on the FDA website that you give you an idea on what they are looking for.

Recall material would be plastic food containers or printed board food containers.

FSANZ Recalls - I quite like that they include pictures, although I guess you wouldn't put on in a letter it is useful in notifications.
