Dear mohsin,

A generic tabular form is simple as far as headings are concerned, eg date / product / quantity / results / satisfactory (Y/N). Examples exist on this forum (somewhere).

But the relevant data entries may not be so simple depending on what you are talking about, eg product / process / criteria for release.

A minimal data entry response for, say FS aspects, could be that for the product lot concerned,  the (Positive Release) form needs to show evidence that all documented FS-related control actions have been carried out and showed no deficiencies with respect to specifications. Or, if deficiencies were observed, these have been validatably corrected as per appropriate documented procedures/forms.

to complete Charles's reply, the decision to release products is heavy in terms of responsibility. you can complete your document with :

- the signature (on paper or via your database) of the person in charge of the release in order to make him fully aware

- insertion of the task in the description job of the person in charge of the positive release

- and ideally 1 procedure explaining the release mentioning that the person responsible of the products remains the boss, but that there is a delegation of its responsibility to the concerned person.

Last it's better not to couple the person in charge of the release and the production manager...possible conflicts of interests

I don't know the size and the organization of your company, so depending on its structure you can keep all or some of the hereabove elements
