Certificate of conformity for polybag used in food packaging

Dear All,
Can someone help me with example of certificate of conformity for poly bags.
I would like to develop a COC for our customers who are FSSC 22000 certified.

it depends which requirements have to be fulfilled. EU, US ...
COC Certificate of Conformity? We are using DoC Declaration of conformity
In the EU the requirements are layed down in the regulation 1935/2004 with regulation 10/2011 for plastics. In the annex the requirements for a declaration of conformity is defined.
