HACCP review document for Storage and Distribution

I noticed in another forum that someone had posted a HACCP meeting record form that was more geared toward food processing. I am looking for something similar skewed toward Storage and Distribution HACCP, specifically for the annual review meeting. Anyone have a document they can provide?
