Procedure to identify the basic needs and expectations of clients

I am looking for a documented procedure to identify the basic needs and expectations of clients.
Start with:
1. New customer questionnaire or the like
2. Any meetings between business development and the client that need to happen
3. Any communication and documents that the client might need to sign (specifications, etc.)
You know your clients better than any of us, so the procedure will be based upon your own process for business development.
