How to conduct a training assessment to verify effectiveness?

I recently had my HACCP audit and i was marked down on Training effectiveness assessment,somebody kindly share how you conduct this exercise.

We usually prepare a questionnaire on the training topic and give it to the participants to fill and see that they understood the topic or not and also check on the job application or implementation of the subject on which the training is conducted. The latter is done by preparing a checklist with all the details of the effectiveness check.

7-10 days after we train some one, we have a member of management sign off on the training document stating the trainee understands their job they were trained in. Then, quarterly, do a Direct Observation of Training.

Train, give comprehension test and keep on file. We keep all physical tests for 3 years and keep a training matrix identifying each topic, trainer, training dates and passed test completion.
