RCA or Cause and Effect Matrix

Tuned into the RCA discussion during this past years FS Magazine conference.

Can I do away with RCA completely if I start using the cause and effect matrix?

Um no you cannot

just because you think you've defined all the cause and effect, something will happen outside of those that you couldn't have thought of

Also, one is preemptive and one is post, you will still have to have a document trail to show what deficiency was identified and what was done to correct AND prevent it
