In my guidelines from CMi it defines an incident as an event that has occured which results in the production or supply of unsafe, illegal or non conforming product.

Please note that these guidelines are for Food not Packaging.

Although I haven't seen any specific guidelines relating to packaging on this issue, the explanation Yorky gives seems valid for packaging also.
