I have tried to fill your attached form somewhat. Hopefully it will help.

As Charles says, since the purpose of this form is unclear (One time activity, internal auditing, external auditing or something else) probably no body will be able to clearly help you out of this .

Also you have not mentioned that whether this form is designed by your own company or you have received it from an external source? Is is mandatory to fill this form? Do you have authority to simplify this form?
