Usually we will be testing savory soups, sauces, gravies and broths. Could also be testing marinades applied to various meat products.
Just looking for a good starting place really.. something that I could tailor to our own needs if need be.

You need to know what kind of information you're looking for. Objective or subjective (Sweet/Salty vs Good/bad). If you're trying to fine tune a product you may want to have different formulas and have the panelist rate them from best to worst or vice versa.
Keep in mind the time available for the panelists to review the product. Panelists generally do not want to answer more than 10 questions.
Remember that we eat with our eyes first: If you're wanting flavor profiles only and there's a color differentiation between products, use a dark room with colored light to eliminate visual aspects.
I took an entire course on Sensory evaluation in college. There's a lot of thought that should go into such testing if you want to get unbiased and real data.
All of this depends on what information you're trying to gain and what the products are.
Here is the book that was used in my college class. It may be a good investment if you're going to commit to sensory projects.
I devised the spreadsheet attached based on the Hedonistic Scale. You can modify the attributes to have ones that are important for you. The sheets given to volunteers don't have the score numbers after the ratings, but you use those numbers to derive a numerical score. You can do averages by volunteer, averages by test item, etc. You can analyze to your heart's content.

I agree with the points in post #4. Case of know yr product.
If it's tasting panel type methods which interests you, in addition to previous post, there are also several quite detailed older threads here with examples of procedures like triangle tests, paired comparisons, etc. some slight searching required.
(TBH i slightly misunderstood yr OP. I had thought you were more seeking a form for inspection of incoming raw materials. :smile: )
