Automatic Failure / Grading of HACCP Audit
Good morning Mentors. Just 2 quick questions. 1. What area(s) / issue(s) should/can in your professional opinion result in an automatic failure of a HACCP or Preventive Control Audit. 2. How vital is it to have a grading system as part of an audit? Is it needed or can a proper audit be conducted without assigning points? If the auditor (external or internal) discovers traces of pests or dead pests (not mentioning alive ones:)) - that's first thing coming to my mind right away

If the written plan is incomplete and/or they aren't documenting that they are following the plan
i love Belize. only been there once though.
here is my opinion. if the program is written as to no provide an adequate level of food safety / control hazards. example - not pasteurizing raw juice or milk.
If the auditor sees a condition were food is being contaminated or will very likely be contaminated. example condensation on a pipe over product, equipment rubbing causing FM in product zone, or significant pest problem as indicated above. some would also consider falsifying documents as well.
You don't have to score audits. however, scoring audits provides trends and scores can be used for goals, kkpis, etc. it also helps those that may not understand the deficiencies a better idea on the condition of the program. So, i always have scored them. Using 1 point deduction for a minor, 10 point deduction for a major, and 25 points for a critical.
A loss of control involving the CCP or PC would be one.
You are going to want to make sure if there are checks in place (example metal detector) that the ones at the CCP/PC know EXACTLY what they are supposed to be doing (properly trained).
If your HACCP plan says at CCP 3 all bags of product must be run through one (1) time on the printed side and one (1) time on the blank side, for a total of two (2) times, but while your customer auditor is there the person at the metal detector runs it through twice on the SAME SIDE; it will not go well (this may or may not have happened to me in the distant past).

LOL. I guess we just like to explore different places. it was probably over 20 years ago. I look at picture of that area and don't really recognize it any more. looks like so much has changed since then. we were in san pedro area most of time, as it was a diving trip. we did spend some time in Belize City and went to the zoo. just tourist stuff.
id definitely go back, heck if the USA continues on their path of lunacy, I might move there.
