Agreements Between Shipper and Carrier

Can anyone supply a template that they have used as an agreement between the shipper and carrier related to service provided by the carrier to the shipper. The agreement should address services as well as insurance and liability at a minimum. I have attached an example however I'm interested in something much more basic.
