In health and safety there is a difference. If someone is hurt it is an accident, if someone nearly got hurt and may have done next time under slightly different circumstances it is an incident. E.g. a hammer dropped from scaffolding that lands on the floor, but cracks no skull is an incident. In food safety I think incident is used when something happens that could put food safety or quality at risk like a glass breakage.
