Pleae provide some context, eg what is approved / verified ? And for what objective / person / standard
Otherwise, IMO, it's unanswerable.
Without knowing the purpose of the form and without seeing it, Approved By is the person who approved the form for use.
Verified By is the person that checks the information on the form to ensure it is complete and accurate.

Without verification, I believe the OP's intent was to ask if someone who "approves" a "form" (which I assume data needs to be recorded on), would also be able to "verify" the data that is recorded on the form.
My answer would be yes, because the person that approved the form is not "verifying" the form, they are "verifying" the data contained on the form.
But then again as stated in my first response, I don't really know what the OP was asking.

my concern is non-confirming sample form, 1 person fill in the form and the other 1 should be verified , rite? Approved by ~ to me is d from or documents being approved before use.

I suspect you are referring to the signing "boxes" required in a Corrective Action form for a haccp system.
The approval box is signed by someone who has authority to release the form into general usage, ie signed before the form is put into use. This is a document control function.
The verify box is signed by the person who is authorised to check the data entered into the form for correctness. This is a haccp control function and will be signed after the appropriate person has checked the data.
The 2 functions are not directly related to each other.
Basically similar to Marshall's posts.
