Do you have an outside company handling your pest control? If so they should be doing the IPM facility assessment for you. We just had our AIB and passed with the assessment that was done by our current pest control company.

David,
We have an outside company handling pest control. But they can not do the IPM facility assessment.
So we're looking this form from AIB.

Thank you very much.

My auditor asked several people during the audit how much of the pest control is done by our pest control service. Most people said all of it while 2 said about 95%. Every single person was wrong. We, the facility personnel, do about 90% of the pest control through all of our prerequisit programs. Examples would be no food or drink in unauthorized areas, no open food kept in lockers, no standing water outside, not putting bug zappers by doors when outside, keeping doors closed and in good condition and all of those things are done daily and all relates to IPM. If your pest service cannot do a facility assessment then you have the wrong service. We use Orkin (hopefully giving the name is allowed?) and they are trained in AIB, IPM and all of that. They do an annual and a quarterly facility assessment which includes review of our IPM program. I got rid of Terminex because they said during an assessment they are told to not look over 8' high. Obviously the top of the dock doors are more than 8' so we would have taken a hit with our auditor. Again, get a service that is trained in AIB and you will be well on your way to success. They even provided free training to the entire Product Safety Committee for IPM to satisfy the federal guidelines. My first year doing this I did the assessment and lost points.

Yes your IPM service provider should complete the assessment, this should be in your contract with the provider as well. The assessment is a review of the last years records and a review of the facility to make any suggestions which will make the program better. It may be adding traps due to activity or it may be as simple as cleaning spills better or fixing a door which has a gap.
