Sorry but parts of the contents of yr attachment look obsolete..

Document was probably intended for iso22000:2005 although some of the content is also debatable (eg decision tree) regarding the earlier version.

 

@chirchir - I suggest you study the Standard iso22000:2018 and the Guidance document iso22004:2014.
