Can anyone share with me their survey questionnaire they hand to the employees? I'm looking to switch the questions up and need ideas. Willing to share mine as well. We are a small group of employees (15)

Your Quiz questions should be based on your training content. Let's say you trained your employees on GMP/GHP. You questions should be reflective of GMP content. You can make questions in the following area as a generic GMP:

1. Facilities cleanliness 

2. Personal hygiene (employee health, safety and lockers)

3. Preventive maintenance

4. Documentation and records

5. Process control (set limits, deviations, and deviation management)

6. Traceability and recall

8. Corrective actions management

9. Employee training (content, trainer, training effectiveness)

10. Self inspection (frequency, who, when, where, how, ...)

 

If your training is on food handling for meeting the food handler card requirements, I would go with basics of food safety. Few areas to consider for your questions may include: 

1. Food safety hazard

2. hand washing

3. temperature control 

4. Cross contamination

5. handling non-conforming products

6. Food allergens

7. Basic hygiene and employee health 

8. Floor organization 

9. Food safety responsibilities 

10. Records requirement
