Hello Everyone,

We recently had an FDSQF audit and auditor asked for a glass & Brittle map (Costco requirement). Our facility don't have or allow any glass and brittle item, which i told to auditor as we don't handle any item-glass&brittle, we're not required to have a map. Still we got the minor, In this case should i appeal or accept the NCR?

Hello and Welcome gatik!

I am not familiar with FDSQF. We have a register (list) of our glass and brittle plastic that we check monthly. I'm surprised that you have no glass or brittle plastic in you facility. What about lighting, dials, computer screens, clocks? Those need to be checked to ensure that fragments don't end up in your product.

We don't have a map, (that would be SO crowded), but the register has satisfied the Costco Addendum for us.

Yes, Costco addendum glass MAP was just brought up at one of our facilities.  We were told it's been there for quite some time

The MAP is to list RISK by area, not to show where each item is  (our at least that is our understanding)

I just used my form 4 to add the risk to each area of the facility

You still need to assign RISK to each area..........so if you only have brittle plastic (lights, and covers as an example) that a map that shows every area is low/negligible risk should suffice

Hello Setanta,

Thank you for response, sorry for confusion but we don't have any production equipments in our facility made from glass and brittle plastic, but we do have a glass and brittle log (list covering lights, machine dials, clocks etc), similarly also checked on monthly basis. I'm sure the SOP and logs were checked during the audit, but still received a minor-NCR. What's our suggestion if to accept or appeal ?

That makes sense. Great thank you for response.
