How does one capture verification of the flow diagram as 2.5.1?

2.6 states that the HACCP team will verify the accuracy of the flow diagram by an on-site audit at least annually. 

We handle this in an documented annual meeting. Once we walk the floor and look at each processing step and verify that the flow diagram is indeed still accurate, then we write that in the meeting notes.

Hi Waciru,
If I understand your question correctly, you use a box on the diagram for signatures of those agreeing to its accuracy and the date of verification.

The precise answer perhaps depends a little on the meaning of yr "capture".

 

Post #2 is also used quite frequently after a confirmatory group walk-through during a given process.

 

IMEX, a fair number of people also do it at least 2 times, the 2nd on the day before the audit. :smile:

 

This is extract from Codex Hygiene 2009 (HACCP segment) -


 

 

 

4.  Construct flow diagram    
The  flow  diagram  should  be  constructed  by  the  HACCP  team  (see  also  “Assemble
HACCP team” above). The flow diagram should cover all steps in the operation for
a  specific  product.  The  same  flow  diagram  may  be  used  for  a  number  of  products
that  are  manufactured  using  similar  processing  steps.  When  applying  HACCP  to  a
given operation, consideration should be given to steps preceding and following the
specified operation.
  5.  On-site confirmation of flow diagram
Steps  must  be  taken  to  confirm  the  processing  operation  against  the  flow  diagram
during  all  stages  and  hours  of  operation  and  amend  the  flow  diagram  where
appropriate. The confirmation of the flow diagram should be performed by a person
or persons with sufficient knowledge of the processing operation.

Agree with above comment, we have a HACCP team meeting at least annually where we walk the floor, agree the diagram is correct and then both document this in the meeting minutes and have the HACCP team leader sign and date the actual plan.
