Question about this form. We do not usually receive materials by trailers -- ours usually unloaded into receiving terminal then transported by our driver to our facility. 

 

The many receiving forms that I see involve trailers.

  1. Is there a version, that doesn't involve trailers?
  2. Or does anyone has a simplified version

You say you don't use trailers, but how is your driver transporting the materials to your facility?  If you're maybe using a box cargo truck of some sort, the same "trailer" inspection criteria would apply (and you could rename your form to "Receiving Checklist" or something similar to account for different vehicles).  You need to be evaluating whether the carry vehicle is clean and sound, no holes in the vehicle that allow pests to hitch a ride into your loading dock.  Verify the material is acceptable and approved for receiving, documenting the description and lot coding, etc.
