Is anyone able to help me with HACCP reassessment form for RTE facility. Also, how do I reassess the plan?

Is the form an internal document, or is it from a third party?

 

Generally speaking, a reassessment is just a review of the policies and other documents to evaluate if they are still applicable, or need to be updated to reflect changes in your process, specifications, or in regulations.

 

The reassessment can be done all on one date, but many programs contain large numbers of documents which encourages setting a schedule of sorts and reviewing some portion of the program each month or fiscal quarter.  Many HACCP programs will include a validation or reassessment schedule that says when or how it is to be performed.

G M's explanation for a reassessment is very good.  To reassess your HACCP Plan, at least once a year, you should reassess the entire HACCP plan and if you don't have any changes then you state it was reassessed and no changes.  I had the opportunity to attend an Advanced HACCP Training several years ago and a couple of forms were shared that I thought were a great tool to help document the entire plan was reassessed.  I am attaching it so you can decide if it might be helpful to you as well.  You can always adjust and modify to make it better for your needs.
