I have a template that would work in either case. I have a check mark for Preventative Maintenance (what I think you're calling shutdown maintenance) and I have a check mark for Corrective Maintenance (what I think you're calling breakdown maintenance). The only difference should be that one is planned while the other is not. In either case the situation should be handled relatively the same. In one case the machine may be done online while the other offline but the procedure would be the same as far as food safety goes. If you agree your SOP should be the following apart from all the maintenance jargon of what needs to be done with the machine.
Tools should be sanitized before working on machine. Machine shall be Locked Out Tagged Out (LOTO). No product in the area while doing maintenance on the equipment. Equipment and tools are rewashed and sanitized by trained personnel. QA verifies work and cleanliness of machine before production begins the use of the machine visually, ATP, swab and signs off. The LOTO is signed off by maintenance who placed the lock and removes it. In some cases the equipment may be placed on HOLD by QA if it doesn't pass inspection.
