Need some help to fill up this form from our client. Hope you can help me find some websites that provide these data. Please see attached file.

(1) Click edit > opens the post for text editing
(2) click "use full editor" at bottom of post > a view which then allows adding attachments (as per usual method)
I asssume you know how to usually add a file as an attachment.?
