Not quite sure what you mean by a HACCP sheet.

 

i assume you are referring to the risk assessment portion of the overall haccp plan, ie hazard analysis et seq

 

The forms required may depend on details required by yr intended standard, eg iso22000 tends to differ from a traditional Codex format.

 

There is no unique design requirement but most HACCP plans contain (a) a hazard analysis form defining the methodology / route to any CCPs, etc.  typically followed by (b) either in text and/or form covering all the other related aspects, eg critical limits, validation, verification etc

 

Yr form appears to be attempting to combine (a,b) in one go. Looks overloaded IMO.

 

As per previous post, probably useful to look through the various templates used by haccp plans on this forum.
