Do you mean a copy of a form ? If so, i don’t have one but i can give you an example of the posssible data required. The corresponding form would be quite simple. (But see the following caveats) -
There may be some terminology confusion -
(1) Validation and verification are both dependent on definition. Sometimes they are used interchangeably or in particular ways.
(2) From a CCP's POV it is more usual to validate / verify the critical limits ??
(3) It is also possible to validate / verify the metal detector’s functionality, eg sensitivity, eg –

(This is also sometimes referred to as [a part of] “calibration” although the latter term may also be seen utilised in activities like “annual calibration”, or [inevitably] “annual validation”).
