Assuming you have an existing HACCP plan, you can use something like the template that I have attached. This is what we use for any new materials or new products. PRIOR to the run, we complete this form and review as a team.
Essentially what it is doing is reviewing raw materials and packaging, and Processing requirements that are new, to see if they will fit under the scope of your current HACCP plan. If they do not, than a hazard analysis must be completed for the item that does not fit.
If you could expand on your situation a little more, I'd be glad to offer more insight. If you are fluent in HACCP and Hazard Analysis' then the form may explain all you need.
