Dear All,

 

Can someone assist me with template of supplier evaluation risk assessment.During FSSC 22000 audit it was found that our Supplier Evaluation was not justified by Risk assessment.

Think about how many different levels of risk you want to define for all of your suppliers.  The example attached above has five levels:  Minimum, Low, Moderate, High and Extreme.

 

Do you want five different sets of controls for five different levels of risk?

 

Level 1 - do A, B and C

Level 2 - do A,B, C and D

Level 3 - do A, B, C, D, E and F

Level 4 - do A-F plus G

Level 5 - do A-G plus H and I

 

Also, in the attached risk assessment, a score of 10 or 12 could be a moderate risk or a high risk depending on likelihood vs. severity (for example 3x4 vs. 4x3).  This may drive your HACCP/Food Safety Team crazy trying to understand the differences.
