I am reviewing inductions and the subject of Medical Questionnaires raises its head again.

 

After discussion with an employment specialist I have to make sure its not discriminatory but at the same time, I need to make food handler / safety decisions.

 

Does any one have an example of a Medical Questionnaire which would be suitable for employees of a Meat Cutting Operation within the UK?

I assume you mean a pre-employment guide.

 

The relevant chapter8 of UK Meat Industry Guide is attached.

 

the attachment (fitness to work) containing questionnaire referred on pg 3  of Chapter 8 is also attached.
