Questions about Root Cause and CAPA

Hello,

I have a basic question about auditing, root cause and CAPA.

 

Shall each point of non-conformity have a root cause, correction and CAPA or when you don't have a system in place you can escape the correction phase and go to CAPA directly ?

 

Is there cases not using root cause analysis for non-conformities? i mean root cause being useless.

There are just some things that are universal in understanding what the immediate correction is, so yes we skip the root a lot as it is not always needed.

There is a famous saying that says "everything happens for a reason". So pretty much every non-conformance has a root cause, there must be a reason WHY it happened. no matter how big or small it is.The only difference is that some of them are easier than others to identify quickly and do not need a lengthy in depth, Root cause analysis investigation. It depends on that non-conformance.
