Food defense is meant to mitigate any risks from intentional harm done to your product, process of facility.
You will start with an risk analysis to asses all the types of risks that can occur. Than you will asses if you have implemented enough measures to mitigate those risks, if not: implement some new measures.
The risk assesment with the measures taken is often referred to as your food defense plan.
There are some good templates you can use:
search from TACCP template, FMEA or FDA Food Defense Builder.
Validate if your food defense plan is still up-to-date at least yearly or when major changes occur.
