Hello, 

 

Can someone please share their new equipment & commissioning checklist. Or can someone look at the one we have and make suggestions on wether it is ok or if you would suggest modifications.

It is for SQF 11.1

I think it looks good.  My only suggestion would be keeping track of any corrections or corrective actions you would have to do based on this.  Such as adding items to your Glass and Brittle Plastic list, etc.  I can honestly say when I have been audited and asked if we have one, the auditor glances at it and moves on to the next section.  The important part here is that you have one in place and it is being utilized.

Thanks for the input.

We recently have had many new & used machines being brought into the facility. As soon as the machines are brought in this form gets filled out by president, general manager and myself. I check for whatever I have to deal with and add them to the appropriate sections of the program like the glass & brittle, SSOP's, etc. Besides that I am not sure how I would keep track if I do not add them to the program right away.

Hi, I found this form here. and thanks for sharing. we produce fish RTE products. I have 3 concerns:

1-Is this form still applicable for us ?

2-Who should fill this out? Supplier company or us?

3-Looks like its only for commissioning, can you also advise some purchasing requirements? what sort of things should we ask the supplier to provide us?

Thanks for your help. Can you also let me know what documents should we ask equipment manufacturer to provide us? is a manual enough to confirm the declarations? and also is there a template to share for a risk based change management ?
