I am looking for an example of Hazard Analysis Worksheet combining Codex Decision Tree and Risk Assessment.  We will be incorporating both the HACCP and HARPC analysis in one worksheet.

Why don’t you draft a template based on CODEX GENERAL PRINCIPLES OF FOOD HYGIENE CXC 1-1969 Table 1: Example of a CCP determination worksheet?

Then add a few columns for an assessment of the hazard to determine the significant hazards.
