I'm drawing a blank on this one. I've used loads of forms in the past to cue you through predelivery assessments of machinery. My brain has gone blank though (Friday feeling much?) All I can think of to check is:

No dirt traps
Quick release for cleaning
Good accessibility for cleaning (all parts can be removed)
Minimal or no clear plastic (no glass)
No white plastic, blue preferably
Sturdy plastics used - not easily abraded

No. If that link doesn't work, try the chilled food association www.chilledfood.org and then go to publications. Some of them are free access and one of those is the hygienic design one.
