I need to create an updated supplier questionnaire forma as the current one is not user friendly and some ten years out of date.

I don't want to make it too long or complex as to put people off filling it in which is an issue with the current one.

If anyone has an example or could give me an indication of what are the key points to include that they could share it would be most appreciated.

In our experience, if you start with the appropriate GFSI standard (BRC Food Safety, IFS Broker etc) checklist and tailor it to you particular requirements you can't go far wrong. You can easily cut it down if you don't think the questions need a response, but make sure that the evidence is provided. Always ask for certificates and audit reports, copies of policies, product specs, HACCP plan etc. This should help you to see if the supplier is being generous with the truth in the SAQ. 

 

Also ensure that the best answer is not always YES on the SAQ as it helps a devious supplier.
