I can't really help you out with pulses or cereals, but the Canadian Food Inspection Agency (CFIA) has this awesome example for spices that was incredibly useful in helping my company develop our HACCP plan years ago and I have used it as a guide in revising the plan, myself.

Obviously your first four forms will be quite different as they are very specific to your operations but 4, 5 and 6 can be very useful in helping determine hazards. I hope this helps!

The US haccp aspect for spices has been discussed in various threads here which often reference the haccp document issued by ASTA which is variously attached on this Forum. Can search a little.

Please note this is a 2-year old thread but thanks anyway.

From a quick look the haccp plan seems quite elegantly structured but operationally rather fudge-like despite it's being quite recent (2017).

There is some similarity to plan in post 2.

Kind Regards,
