Does the PCQI need to sign off on all the sanitation logs or just when an allergen has been cleaned & tested (3D Reveal)? We currently use a lab service for our COA's. Does the PCQI need to sign off on the COA's? Do we need written documentation of this?

Yes to all of the above. Maintain these sanitation records and have them reviewed by the PCQI.
