Hello everyone,

 

I am creating a new Supplier Approval Program and have created a Food Safety and Quality Assurance Supplier Questionnaire.

 

I am very new to this and am feeling uncertain of my questionnaire. If anyone can look it over and tell me what they think, I would be extremely grateful!

 

Please critique me and give me all the comments you can, I am doing this alone as a first time QA Manager and want to make sure it is up to par.

I would remove the first section, they don't need all of that information; as well as your document control at the top (keep them for your word doc, remove for the pdf you'll send)

 

Don't be surprised if you get a lot of pushback on audit reports and your request for HACCP plans etc as lots of people will not provide them 

 

Other than that, you've covered all the bases

Is there standard you are audited against?

As Scampi mentioned, many companies do not want to share certain information (audit reports with corrective actions, HACCP plans, etc) so you may want to be careful in creating your supplier approval program. If you require these items but are unable to get them from every supplier, then you won't be following your own program, unless you write in that they are not required in order for a supplier to be approved.
