First you need a "change agent" someone willing and empowered to take on this responsibility.

Here are 3 excellent resources for change management:
There are major sections in each of these books that outline Change Management.

1. "The Certified Manager of Quality/Organizational Excellence Handbook", 3rd edition, Russell T. Westcott - find at ASQ.org
2. "Lean thinking", James P. Womack and Daniel T. Jones - see chapter 11, Action Plan
3. "The six sigma handbook", Pyzdek

I am also attaching an ASQ presentation from Steve Gerhardt, Creating a Culture of Quality -
